Eligibility Requirement
- An individual between the ages of 18-59 who is mentally alert and has a physical disability for one year or longer Or
- An individual 60 and over who meets the level of care for a skilled nursing facility

- Assisting patients with medical recovery at home when temporary or acute physical conditions occur
